Established in 2014, the LifeBridge Health Accountable Care Organization strives to further improve the care of Medicare patients and the health of populations while simultaneously reducing per capita costs.
Advanced Health Collaborative
Advanced Health Collaborative is a voluntary collaboration between LifeBridge Health and other independent health systems in Maryland to share ideas and find ways to efficiently and effectively improve health care, resulting in better quality at reduced cost.
LifeBridge Health has partnered with more than 500 primary care physicians and specialists practicing in the community and giving patients the care they need — right in their neighborhood. From pediatrics to geriatrics, LifeBridge Health has a physician for you.
ExpressCare Urgent Care Centers provides high quality walk-in urgent care for patients of all ages, with evening and weekend hours; onsite X-ray, lab and prescription services; and 20 locations throughout the mid-Atlantic.
Our Medicare-certified home health care agency provides specialized care services — skilled nurses, physical and occupational therapists, dietitians, medical social workers and home health aides — to homebound patients in the region.

Powered by Physiotherapy Associates, LifeBridge Health Physical Therapy offers back and neck care, postoperative and work injury rehabilitation, hand therapy, aquatic therapy, neurological rehabilitation, and other specialty services at several locations throughout Baltimore.
National Respiratory Care
Working primarily with LifeBridge Health facilities and physicians, National Respiratory Care provides and installs respiratory and hospital equipment for homebound patients. National Respiratory Care will also send a technician to assist patients with equipment maintenance and usage.
The dedicated pharmacists at Sinai and Northwest hospitals provide patients with automated refills, same-day pickup, and free bedside and home delivery service. Our pharmacies offer competitive prices and accept most insurance providers.
Post-Acute Physician Partners
Post-Acute Physician Partners supplies post-acute care providers to chronic hospitals; subacute, skilled nursing, long-term care, assisted living and independent living facilities; and home care services. PAPP providers spend 100 percent of their time practicing in the post-acute setting, which improves the quality of their care and lowers the cost of that care.
Pulse Medical Transportation
Our critical care transport company offers emergency, non-emergency and wheelchair transportation to patients with a variety of needs, 24 hours a day, seven days a week.
